DAPTOMYCIN
Daptomycin includes the CLSI recommended calcium supplementation. No additional supplementation is required.
LIMITATION OF THE PROCEDURE
1. MicroScan Gram Positive panels should not be used to determine the susceptibilities of Streptococcus except S.
agalactiae (Group B) and S. bovis group. These isolates should be tested using an accepted method such as the
MicroScan MICroSTREP plus Panel.
2. CLSI recommends supplementation of Mueller Hinton Broth with Lysed Horse Blood when testing fastidious streptococci
(CLSI Document M07) and L. monocytogenes (CLSI Document M45).
Procedure differs from this recommendation. If there is inadequate growth in the growth well, the MIC results from S.
agalactiae (Group B), S. bovis group, and L. monocytogenes are not valid and an alternative method should be used.
3. The MicroScan Dried Gram Positive Panel may be used to identify fastidious streptococci. If there is inadequate growth
in the growth well, the biochemical results are not valid and an alternate method should be used.
4. Anaerobic Bacteria - The Pos Combo/Pos MIC panels are not suitable for testing anaerobic gram positive cocci.
5. Additional tests may be necessary to determine the final identification when a low probability (<85%) identification is
obtained; refer to the Microbiologics Manual.
6. The penicillin MICs for coagulase negative staphylococci may not be useful in predicting the resistance due to
β-lactamase. Therefore, a β-lactamase test should be performed to confirm the MIC. Strains of S. saprophyticus with
penicillin MICs of 1-2 μg/mL have been shown not to produce β-lactamase and thus ampicillin, amoxicillin or penicillin
may be the drug of choice for urinary tract infections with these organisms.
7. A slight haziness may occur in random wells of some antimicrobics due to the incomplete solubility of some media
constituents. This should not be interpreted as growth.
8. The interpretation of test results requires trained clinical personnel who should use judgment, knowledge and additional
confirmatory tests where required prior to accepting the identification of an organism.
9. Biotype numbers should not be used for phenotype identification of strains isolated from various specimens from the
same patient.
10. Results obtained with the following organism/antimicrobial agent combinations listed below have shown discrepant MICs
when compared with an overnight reference method. If the antimicrobial agent is critical to patient care, an alternative
procedure should be used or the antimicrobial agent should not be reported due to low correlation.
