(thyrocalcitonin) in human serum or
heparinized plasma, as an aid in the
diagnosis and treatment of diseases
involving the thyroid and parathyroid
glands, including carcinoma and
hyperparathyroidism.

Summary and Explanation
Human calcitonin is a single-chain peptide
hormone produced primarily in the thyroid
gland, where it is secreted by the
parafollicular "C" cells rather than by the
thyroxine-secreting cells.
chain is 32 amino acids in length, and has
a molecular mass of 3418 daltons.
pathological conditions, the hormone
exhibits a certain degree of heterogeneity.
Its half-life in plasma is approximately 10
minutes. Metabolized predominantly by
the kidney, calcitonin is also degraded to
some extent by a plasma factor, making it
important to freeze samples soon after

collection.
The physiological significance of calcitonin
is uncertain, but it does have a modest
calcium- and phosphorus-lowering effect.
Its secretion is normally modulated by
rising and falling calcium levels, with an
increase in circulating calcium prompting
an increase in the calcitonin level.
A fasting basal range of 3–19 pg/mL for
healthy adults, based on
radioimmunoassay of extracted plasma,
has been reported.
(greater than 100 pg/mL) are encountered
in a variety of pathological conditions,
notably medullary thyroid carcinoma, a
tumor of the calcitonin-secreting cells of
the thyroid. The peptide is also frequently
elevated in leukemic and
myeloproliferative disorders.

In addition, it may be produced ectopically
by tumors, e.g. those of the lung or

breast.
Its measurement,
however, does not appear to be of value
in the diagnosis of such disorders.
Elevations may also be seen in
connection with hyperparathyroidism,
hypergastrinemia, renal failure and chronic
inflammatory disease.
Principle of the Procedure
IMMULITE 2000 Calcitonin is a
solid-phase, enzyme-labeled, two-site
chemiluminescent immunometric assay.
Incubation Cycles: 2 × 30 minutes
Time to first result: 65 minutes
Specimen Collection
A fasting morning specimen is usually
obtained to establish baseline values. If
the samples are being collected as part of
a provocative test, the time of collection
should be noted relative to calcium or
pentagastrin infusion.
EDTA has an effect on the assay and is
not recommended as a sample type.
The use of an ultracentrifuge is
recommended to clear lipemic samples.
Hemolyzed samples may indicate
mistreatment of a specimen before receipt
by the laboratory; hence the results should
be interpreted with caution.
Centrifuging serum samples before a
complete clot forms may result in the
presence of fibrin. To prevent erroneous
results due to the presence of fibrin,
ensure that complete clot formation has
taken place prior to centrifugation of
samples. Some samples, particularly
those from patients receiving
anticoagulant therapy, may require
increased clotting time.
Blood collection tubes from different
manufacturers may yield differing values,
depending on materials and additives,
including gel or physical barriers, clot
activators and/or anticoagulants.
IMMULITE 2000 Calcitonin has not been
tested with all possible variations of tube
