cause hazards for persons, environment and special tool,

o hazards to persons caused by unsecured work
areas,
o failure of important functions of the special tool,
o failure of specified methods for the maintenance
and repair,
o hazard to persons caused by mechanical and
chemical effects,
result in the voidance of any claims for compensation.
