FORKLIFT LIFTING HOOK
WARNING! Read these instructions carefully
before using.
• Every person who installs, operates or maintains
this attachment must read and understand these
instructions prior to use.
• Do not exceed rated weight while using.
• Improper use could result in death or injury and/or
property damage.
• Inspect before use. Do not operate if lifting hook is
malfunctioning or damaged.
• Do not modify the lifting hook in any way.
• Do not use to lift people or items over people.
• Ensure all personnel remain clear of lifting hook
when in use.
• Only lift the load as high as needed. Keep close to
the ground.
• Use the safety chain to secure the lifting hook to
the fork truck carriage.
• Do not operate if the safety chain is missing or
damaged. The safety chain is designed to prevent
slippage and will not support a full rating load.
• Verify the fork mast is tilted back toward the
operator. Never tilt the mast top forward, as lifting
hook slippage may occur.

SAFETY
• Improper rigging practices may result in injury
and/or property damage.
• Use properly rated lifting equipment when lifting
a load with the lifting hook.
• Use the lifting hook on a level surface, avoiding
sudden starts, stops or turns.
• Verify the forklift parking brake is engaged before
dismounting the truck when the lifting hook is
attached.
• Only use lifting hook when attached properly to
a forklift.
• Use for lifting only. Lifting hooks are not suitable for
dragging material.
• Evenly distribute load. Do not use on a load that is
unstable.
• Do not leave an elevated load unattended.
• Do not attempt to lift a load that weighs more than
the rated load of the attachment or the forklift,
whichever is smaller.
• Ensure the locking pins are tight before all
operations.
